Team Leader (Health Care), Co. Kildare/Meath, Job Ref. JO3190. Enfield. Contract. Salary €22.67 per hour. Hours are fulltime. We have partnered with an established charity in Ireland who support individuals with an intellectual disability, autism and those in need of mental health services. Due to continued growth and expansion as a provider of quality services, they wish to recruit a Team Leader for a site in Co. Kildare/Meath. This is a fantastic and exciting opportunity to be part of a growing and innovative organisation.Main Function: To assist the Manager of Services/Deputy Manager in the effective and efficient management of the services, including outreach, day care, supported living and residential services, and to lead the team alongside the Manager of Services/Deputy Manager in providing the highest possible standard of support to Service Users in line with company strategy. This post is designed to meet the demands of delivering quality services to adults, with Autism and other complex needs. The service at this site has particular challenges as a result of its campus setting, this requires Team Leaders that are able to assume significant ownership and responsibility for their allocated area of concern. Team Leaders will be expected to liaise effectively with the Multi-Disciplinary team ensuring that a comprehensive assessment of need is in place along with the resultant support plan. The Team Leader will also assume responsibilities to assist the Manager of Services/Deputy Manager in ensuring the services meet the regulatory responsibilities of the company.Main duties and responsibilities: This description is neither exhaustive nor exclusive and may be reviewed and amended in the future to include any other reasonable duties, projects or tasks as may be requested from time to time and to reflect changing organisational requirements, changing staffing levels, etc. A full job description can be provided to shortlisted applicants. Develop & maintain a trusting & professional relationship with Service Users and act in a supportive & empathetic manner towards them. Contribute to & help maintain a harmonious and therapeutic environment. Support Service Users to manage their physical health, mental health and wellbeing through the development, implementation and monitoring of individual care plans. To provide outreach support to service users in a home environment to support Service Users with identified needs. Actively encourage the involvement of Service Users in all aspects of decision making, be sensitive and seek solutions to difficulties encountered by Service Users. Implement & adhere to Policy and Procedures for Referral & Living in Residential Housing involving Service Users at all times. Deliver care & support within the context of a social inclusion and recovery approach which is designed to work with people to find and maintain hope, re-establish a positive identity, build a meaningful life and take responsibility and control. To adhere to the Safe Handling, Administration, Storage and Control of Medication as relevant to your particular Services, subject to training and assessment of competence. Managing/supervising Co Workers to ensure they provide care and support by motivating and mentoring Service Users. Be flexible and work such hours to meet the needs of the Services as are required for the post which involves evening, sleepovers and weekend work on a shift/rota basis. Staff are required to have business class insurance on their own vehicles for the transporting of service users during working hours and travelling between different services. Travel to, and work at, other locations as required. Support the Manager of Services/Deputy Manager to organise the provision of support for Residents including; staff rotas and the allocation of duties and key worker responsibilities. Convene and minute partnership meetings ensuring relevant information is shared. To assist the effective financial management of resources and review local practice across key cost areas. Take responsibility for financial management for duration of shift and ensure implementation of policy and procedure systems at handover. To Develop and complete regular reviews and evaluate all aspects of care and support delivered to demonstrate outcome attainment. Staff are required to establish and maintain relationships with people with Autism that are based on respect and equality and that promote their independence. The Team Leader will endeavour to ensure the on-going confidence of the public. The Team Leader will maintain high standards of personal accountability.Key requirements: Essential Criteria. Professional qualification for example: Professional Social Work qualification, Qualified Intellectual Disability/Mental Health Nurse, A qualified Health Care Professional. AND a minimum of 2 years’ experience working in a Social Care setting. OR. A relevant Social Care Degree or equivalent related qualification AND a minimum of 2 years’ experience working in a Social Care setting. OR. QQI/FETAC Major Level 5 or equivalent in Healthcare or equivalent related qualification AND. A minimum of 3 years’ experience working in a Social Care setting. Experience of liaising with statutory and other voluntary agencies. Proven experience of financial planning and managing Service Users monies. Have proven experience of motivating Service Users. Have proven experience developing care and support plans and implementing programmes with Service Users. You will also be required to be flexible as the role requires 24 hours a day, 7 days a week cover. This can involve sleep over cover, night cover, evening and weekend work on a shift rota basis. You should also possess a full current driving licence that allows you to drive in ROI and access to a car with appropriate business insurance will be a requirement for this post. Consideration will be given to alternative travelling proposals in respect of applicants with a disability who cannot hold a driver’s licence. This is a regulated post and will be subject to a satisfactory Garda Síochána Vetting.
